1.护照原件，最少有六个月有效期，并有一张有效的签证空白页。

3.照片两张。2寸，近期，正面（不露齿），彩色（白色背景），免冠护照照片。
4.如有美国绿卡，要提供复印件；如果是其它签证，要提供相关证明。（比如I20复印件， I797复印件等）
5.从本城市飞往香港的机票行程单和酒店预订单，第一次入境是在香港，转机可以在其它城市。
6.如果是商业访问，需要现在申请人现在工作的公司出具一封证明信。
